1. Original High School Report Card( Form 128) and one Photocopy of the same
2. Photocopy of Birth Certificate
3. Certificate of Good Moral Character signed by the Principal/Registrar or duly authorized representative from the school last attended.
4. 2 copies of 2×2 ID Pictures
5. Duly accomplished Medical Examination from University Medical/ Dental Clinic
6. 1 brown envelop
(For Transferees)
1. Certificate of transfer Credential from the school last attended
2. Informative copy of transcript of records
3. Photocopy of Birth Certificate
4. Certificate of Good Moral Character signed by the Principal/Registraror duly authorized representative from the last attended
5. 2 copies of 2×2 ID Pictures
6. Duly accomplished Medical Examination from University Medical/Dental Clinic
7. 1 brown envelop
